Eligibility Criteria (আবেদনের যোগ্যতা)

  • Permanent Resident: Must be a resident of West Bengal.
    (পশ্চিমবঙ্গের স্থায়ী বাসিন্দা হতে হবে।)
  • Ration Card: Must have a Digital Ration Card.
    (ডিজিটাল রেশন কার্ড থাকা বাধ্যতামূলক।)
  • Bank Account: Family head must have a bank account for DBT.
    (সরাসরি টাকা পাওয়ার জন্য পরিবারের প্রধানের ব্যাঙ্ক অ্যাকাউন্ট থাকতে হবে।)

Required Documents (প্রয়োজনীয় নথিপত্র)

Keep these documents ready before filling the form: ফর্ম পূরণ করার আগে নিচের নথিগুলি সাথে রাখুন:
  • Aadhaar Card: For all family members. (সকল সদস্যের আধার কার্ড)
  • Voter ID (EPIC): For members above 18 years. (ভোটার কার্ড)
  • Ration Card: Copy of Digital Ration Card. (ডিজিটাল রেশন কার্ড)
  • Bank Passbook: With IFSC Code & Account No. (ব্যাঙ্ক পাসবুক)
  • Land Records: Property details or ROR. (জমির পর্চা বা খতিয়ান)
  • Income Proof: Yearly family income info. (বার্ষিক আয়ের প্রমাণ)

How to Fill the Form? (কীভাবে ফর্ম পূরণ করবেন?)

Step 1: Fill in the Head of Family (HOF) details like Name, Aadhaar, and Address.
ধাপ ১: পরিবারের প্রধানের নাম, আধার এবং ঠিকানা দিয়ে ফর্ম শুরু করুন।

Step 2: Enter details of all other members including education and occupation.
ধাপ ২: পরিবারের অন্যান্য সদস্যদের শিক্ষা ও পেশার বিবরণ দিন।

Step 3: Provide correct Bank details (IFSC & Acc No) to receive benefits.
ধাপ ৩: সরকারি সুবিধা পাওয়ার জন্য সঠিক ব্যাঙ্ক অ্যাকাউন্ট নম্বর ও IFSC কোড দিন।
